Patient Forms

Before your appointment, please review the menu to conveniently access our locations, driving directions, patient forms, office policies, and insurance information. If you wish, you can download and complete your patient registration form prior to your visit. Please bring your completed form, insurance cards, list of medications, and copies of relevant medical records to your appointment. 

If you wish to schedule an appointment at one of our offices, please call the number listed for that location.

Your request will be processed within 24 hours, during normal business hours, Monday – Friday, 9:00 am – 4:30 pm, excluding holidays. If you do not receive confirmation of your request within 48 hours, please contact our office.

Telephone Procedures

During Office Hours:  Our number is answered by the staff, who will ask your name, phone number, and the nature of your call. Your message will be relayed to the appropriate healthcare professional.

Emergencies:  In the case of an emergency please call 911. 

After Hours:  In the case of an emergency, please call 911. Calls for routine questions and prescription refills should be made during office hours. For concerns after hours, please call the office and our answering service will relay your message to the emergency cardiologist on call. The on call cardiologist will return your call as quickly as possible. If your phone has ‘anonymous call rejection’ please hit *87 to deactivate this feature. After receiving the call from the cardiologist, this feature may be reactivated by pressing *77.
